Ice Cream Cone Sign in ENT
The Ice Cream Cone Sign is an important radiological sign encountered in ENT.
Interestingly, there are two different Ice Cream Cone Signs:
- A normal anatomical finding seen on CT Temporal Bone.
- A pathological finding seen in Acoustic Neuroma (Vestibular Schwannoma) on MRI.
In this post, we will discuss both of them in detail.
Ice Cream Cone Sign in Normal Temporal Bone
๐ Type of Sign
๐ Radiological sign
Seen on CT Scan of Temporal Bone.
๐ What is the Sign?
The normal ossicular anatomy creates an appearance resembling an ice cream cone.
-
Ice cream Scoop = Head of Malleus
-
Ice cream Cone = Body of Incus
Together they produce the characteristic Ice Cream Cone Appearance.
๐ Where is it Seen?
Seen on Axial CT section of the Temporal Bone.
Specifically at the level of Epitympanum.
๐ง Why Does it Occur?
The appearance is produced by:
- Ice cream Scoop is formed by the Head of Malleus
- Ice cream Cone is formed by Body of Incus
The tapering part of the cone is formed by Short process of incus which points toward the Aditus ad Antrum.
โญ Clinical Importance
The normal Ice Cream Cone appearance confirms:
โ Intact Incudomalleolar Joint
Loss of this appearance suggests Incudomalleolar Disarticulation
Ice Cream Cone Sign in Acoustic Neuroma
๐ Type of Sign
๐ Radiological sign
Seen on MRI Brain.
๐ What is the Sign?
An Acoustic Neuroma (Vestibular Schwannoma) produces a characteristic Ice Cream Cone appearance.
Components
-
Ice cream Scoop = Tumor Component in Cerebellopontine Angle (CPA)
-
Ice cream Cone = Tumor Component in Internal Auditory Canal (IAC)
Together they form the characteristic Ice Cream Cone Sign.
๐ฆ Seen In
Vestibular Schwannoma (Also called Acoustic Neuroma)
๐ง Why Does it Occur?
-
Vestibular Schwannoma arises from Schwann Cells of the Vestibular Nerve.
-
The tumor originates within the Internal Auditory Canal.
-
As it enlarges, it extends outward into the Cerebellopontine Angle.
This produces Narrow intracanalicular component (cone) & Expanded CPA component (scoop), creating the Ice Cream Cone appearance.
๐ฆ Why is it Called Ice Cream Cone Sign?
The tumor resembles:
- A scoop of ice cream in the CPA
- Sitting on a cone within the Internal Auditory Canal
Hence the name Ice Cream Cone Sign.

โญ High Yield Rapid Revision Table
| Ice Cream Cone Sign | Type | Components | Seen In |
|---|---|---|---|
| Normal Ice Cream Cone Sign | Radiological | Head of malleus + Body of incus | Normal CT Temporal Bone |
| Pathological Ice Cream Cone Sign | Radiological | CPA component + IAC component | Vestibular Schwannoma |
โก Normal vs Pathological Ice Cream Cone Sign
| Feature | Normal Ice Cream Cone Sign | Acoustic Neuroma Ice Cream Cone Sign |
|---|---|---|
| Imaging | CT Temporal Bone | MRI Brain/IAC |
| Structure Forming Scoop | Head of Malleus | CPA Tumor Component |
| Structure Forming Cone | Body of Incus | IAC Tumor Component |
| Significance | Normal Anatomy | Vestibular Schwannoma |
| Clinical Importance | Assesses ossicular integrity | Diagnoses CPA tumor |
๐ง Viva Pearls
- Ice Cream Cone Sign has both normal and pathological meanings in ENT.
- Normal Ice Cream Cone Sign is formed by the head of malleus and body of incus, seen on axial CT temporal bone at the level of the epitympanum.
- Loss of normal Ice Cream Cone Sign suggests incudomalleolar disarticulation.
- Vestibular Schwannoma produces a pathological Ice Cream Cone Sign on MRI.
- Best investigation for Acoustic Neuroma is MRI with gadolinium contrast.
