๐Ÿ‘ƒ Kuhn's Classification of Frontal Cells

๐Ÿ“ Definitions of Frontal Cells

YearAuthorDefinition
1941Van AlyeaCells encroaching on the frontal recess or the frontal sinus
1994Bent and KuhnAnterior ethmoidal cells arising from behind and above the agger nasi, pneumatizing the frontal recess

โญ Frontal cells are anterior ethmoidal cells.

๐Ÿ“‹ Original Kuhn's Classification

โญ Kuhn classified frontal cells into four types, based on the anatomical location of the cell in relation to the agger nasi cell.

๐Ÿ‘‰ The first step on CT is always to locate the agger nasi cell.

TypeDescription
Type 1A single frontal recess cell lying above the agger nasi
Type 2A tier of cells in the frontal recess, lying above the agger nasi
Type 3A single massive cell pneumatizing cephalad into the frontal sinus
Type 4A single isolated cell lying within the frontal sinus

Which cells got excluded from Kuhn's Classification?

๐Ÿ“‹ Modified Kuhn's Classification (Wormald)

โญ Wormald modified only Type 3 and Type 4.

๐Ÿ‘‰ The modification introduced the vertical height of the frontal sinus as the criterion, assessing how far the cell extends into the frontal sinus.

TypeDescription
Type 1A single frontal recess cell lying above the agger nasi (unchanged)
Type 2A tier of cells in the frontal recess, lying above the agger nasi (unchanged)
Type 3A single cell extending into the frontal sinus, occupying less than 50% of its vertical height
Type 4A single cell extending into the frontal sinus, occupying more than 50% of its vertical height

Type 4Single isolated cell within the frontal sinusSingle cell occupying more than 50% of the vertical height of the frontal sinus

๐Ÿ“‹ IFAC Classification

The International Frontal Sinus Anatomy Classification (IFAC) is the classification currently followed, as it is based on anatomical origin.

What is the IFAC classification of frontal cells?

  • Frontal cells are anterior ethmoidal cells.
  • Van Alyea in 1941 defined frontal cells as cells encroaching on the frontal recess or frontal sinus.
  • Bent and Kuhn in 1994 defined them as cells arising from behind and above the agger nasi, pneumatizing the frontal recess.
  • Kuhn's classification is based on the relation of the cell to the agger nasi cell.
  • The first step on CT is to locate the agger nasi cell.
  • Type 1 is a single cell above the agger nasi.
  • Type 2 is a tier of cells above the agger nasi.
  • Wormald modified only Type 3 and Type 4, using the vertical height of the frontal sinus.
  • Modified Type 3 occupies less than 50% of the vertical height of the frontal sinus.
  • Modified Type 4 occupies more than 50% of the vertical height of the frontal sinus.
  • IFAC is the classification currently followed, and is based on anatomical origin.
