Bocca Sign

👉 Clinical Sign elicited during Palpation of the neck.

🔍 What is the Sign?

Bocca Sign refers to the absence of laryngeal crepitus on palpation of the larynx.

The larynx feels fixed or immobile.

🩺 How is it Elicited?

The larynx is moved from side to side over the cervical spine.

What is Muir's crackle?

🧠 Why Does it Occur?

Loss of the retropharyngeal space → Larynx cannot glide freely over the vertebral column → Absence of laryngeal crepitus

Seen In

Conditions that obliterate the retropharyngeal space:

1- Malignancy — Post-cricoid carcinoma (Most classical association), due to infiltration or fixation

2- Infections & Abscesses — Pre-vertebral abscess, Retropharyngeal abscess

3- Lymphatic & Mass Lesions — Retropharyngeal tumors, Retropharyngeal lymphadenopathy

4- Tuberculosis of the Spine — Pott's spine involving the cervical vertebrae

Named after Bocca, who described the diagnostic significance of laryngeal crepitus in post-cricoid and retropharyngeal pathologies.

Boyce Sign

👉 Clinical Sign elicited during Palpation of the neck.

🔍 What is the Sign?

Boyce Sign refers to an audible gurgling or splashing sound on palpation of the neck, due to fluid and air in a pharyngeal pouch.

Seen in Zenker's Diverticulum (Pharyngeal Pouch).

🧠 Why Does it Occur?

• Food and fluid collect inside a large pharyngeal pouch
• On palpation of the neck, usually on the left side, the pouch contents get displaced
• The presence of both food and fluid produces a gurgling or splashing sound

Named after Boyce, who described the characteristic gurgling sound produced by retained fluid inside the pharyngeal pouch.

👉 Clinical Sign elicited during Neck examination.

🔍 What is the Sign?

Bryce Sign refers to a hissing sound produced on manual compression of an external laryngocele, due to escape of air into the larynx.

🧠 Why Does it Occur?

• Raised intralaryngeal pressure fills the laryngocele with air
• An external laryngocele is an air filled sac communicating with the laryngeal lumen
• On compression of the swelling, air escapes back into the airway lumen

Producing a hissing sound.

Seen In

✅ Combined laryngocele

❌ Not seen in Internal laryngocele
