๐Ÿ”„ Decannulation (Removal of Tracheostomy Tube)

Decannulation is the process of removing the tracheostomy tube once the patient no longer requires it.

It is the final step in weaning a patient from tracheostomy.

โ— Why is Decannulation Important?

Tracheostomy tube should not be retained longer than necessary because prolonged use can lead to:

  • Tracheal ulceration
  • Increased tracheobronchial secretions
  • Granuloma formation
  • Tracheal stenosis
  • Unsightly scar

๐Ÿ‘‰ Therefore, timely decannulation is essential.

โœ… Indications for Decannulation

1๏ธโƒฃ Resolution of primary pathology

  • Upper airway obstruction resolved
  • No longer need for airway protection

2๏ธโƒฃ Normal protective laryngeal reflexes

  • Adequate cough reflex
  • No aspiration during swallowing

3๏ธโƒฃ No planned further airway intervention

  • No upcoming head & neck surgery
  • No planned radiotherapy

4๏ธโƒฃ No requirement of mechanical ventilation

What is the minimum clinical criteria Before Decannulation?

What are the Investigations done before Decannulation?

โš™๏ธ Decannulation Procedure (Stepwise)

Step 1๏ธโƒฃ โ€“ Deflate Cuff

  • Assess patient tolerance

Step 2๏ธโƒฃ โ€“ Change to Uncuffed Tube

  • Reassess tolerance

Step 3๏ธโƒฃ โ€“ Tube Blocking Trial

  • Block tube during daytime
  • Keep open at night (first 24 hrs)

Step 4๏ธโƒฃ โ€“ Full Occlusion Trial

  • Occlude tube completely for 24 hrs

๐Ÿ‘‰ If not tolerated โ†’ Downsize tube

Step 5๏ธโƒฃ โ€“ Tube Removal

  • Remove tracheostomy tube
  • Apply airtight occlusive dressing

๐Ÿฉน Tracheostomy Stoma Healing

  • Usually heals within ~2 weeks
  • Dressing changed if air leak present
  • If persistent opening โ†’ Secondary surgical closure (2-layer suturing)

๐Ÿง  Psychological Dependence

  • Long-term tracheostomy patients may develop anxiety about breathing without tube
  • Decannulation may require gradual approach

โ— Difficult Decannulation

๐Ÿ“Œ Definition of Difficult Decannulation

Failure to tolerate tube removal or need for reinsertion due to:

  • Respiratory distress
  • Airway obstruction
  • Secretion retention

What are the causes of Difficult Decannulation?

๐Ÿ‘ถ Precautions During Decannulation in Children

  • Perform decannulation in OT

  • Keep re intubation equipment ready

    • Headlight
    • Laryngoscope
    • Proper sized Endotracheal Tube
    • Tracheostomy tray
  • Do pre-decannulation endoscopy

What should you check during endoscopy before planning for a decannulation?

  • Observe child for several hours post-decannulation
    • Respiratory distress
    • Tachycardia
    • Oxygen saturation monitoring
    • ABG if required

โญ Exam Pearls

  • Decannulation = Final step of tracheostomy weaning
  • Strong cough reflex + patent airway = Key requirements
  • Most common cause of difficult decannulation โ†’ Persistent pathology
  • Psychological dependence โ†’ Important clinical cause
  • Children โ†’ Always decannulate in OT
