# Decannulation

## 🔄 Decannulation (Removal of Tracheostomy Tube)  

**Decannulation** is the process of **removing the tracheostomy tube** once the patient no longer requires it.

It is the **final step in weaning a patient from tracheostomy.**

### ❗ Why is Decannulation Important?

Tracheostomy tube should **not be retained longer than necessary** because prolonged use can lead to:

- Tracheal ulceration  
- Increased tracheobronchial secretions  
- Granuloma formation  
- Tracheal stenosis  
- Unsightly scar  

👉 Therefore, **timely decannulation is essential.**

### ✅ Indications for Decannulation

1️⃣ **Resolution of primary pathology** 

- Upper airway obstruction resolved  
- No longer need for airway protection  

2️⃣ **Normal protective laryngeal reflexes**

- Adequate cough reflex  
- No aspiration during swallowing  

3️⃣ **No planned further airway intervention**

- No upcoming head & neck surgery  
- No planned radiotherapy  

4️⃣ **No requirement of mechanical ventilation**

### ⚙️ Decannulation Procedure (Stepwise)

**Step 1️⃣ – Deflate Cuff**

- Assess patient tolerance

**Step 2️⃣ – Change to Uncuffed Tube**

- Reassess tolerance

**Step 3️⃣ – Tube Blocking Trial**

- Block tube during **daytime**
- Keep open at night (first 24 hrs)

**Step 4️⃣ – Full Occlusion Trial**

- Occlude tube **completely for 24 hrs**

👉 If not tolerated → **Downsize tube**

**Step 5️⃣ – Tube Removal**

- Remove tracheostomy tube
- Apply **airtight occlusive dressing**

### 🩹 Tracheostomy Stoma Healing

- Usually heals within **~2 weeks**
- Dressing changed if air leak present
- If persistent opening → **Secondary surgical closure (2-layer suturing)**

### 🧠 Psychological Dependence

- Long-term tracheostomy patients may develop **anxiety about breathing without tube**
- Decannulation may require **gradual approach**

### 📌 Definition of Difficult Decannulation

Failure to tolerate tube removal or **need for reinsertion** due to:

- Respiratory distress  
- Airway obstruction  
- Secretion retention

- Perform **decannulation in OT**
- Keep **re intubation equipment ready**
  - Headlight  
  - Laryngoscope  
  - Proper sized Endotracheal Tube  
  - Tracheostomy tray  

- Do **pre-decannulation endoscopy**

- Observe child **for several hours post-decannulation**
  - Respiratory distress  
  - Tachycardia  
  - Oxygen saturation monitoring  
  - ABG if required
