๐ฌ Narrow Band Imaging (NBI) in ENT
๐ Definition of Narrow Band Imaging
Narrow band imaging is an optical image enhancement endoscopic technique that uses filtered light of specific wavelengths instead of white light.
๐ This enhances the visualization of the superficial mucosal and submucosal vascular patterns.
What are the wavelengths of filtered light used in Narrow Band imaging?
๐ฏ Purpose
Early detection and characterization of premalignant and malignant lesions of the upper aerodigestive tract.
What are the differences between Narrow Band Imaging and Contact Endoscopy?
๐ฐ๏ธ History of Narrow Band Imaging
- First developed in the early 2000s by Olympus Medical Systems
- Initially used in gastrointestinal endoscopy
- Later adapted to ENT, especially for the larynx, oral cavity and pharynx
- Has now rapidly gained importance in early cancer detection protocols
๐ Principle of Narrow Band Imaging
Based on the optical filtering of white light into specific wavelengths.
๐ The equipment uses filters that emit only narrow bands of blue and green light.
โญ Narrow band imaging enhances the existing vascular patterns without the use of dyes.
Why are Blue and Green Light only used in Narrow Band Imaging?
Which structures are visualised using Blue light in Narrow Band Imaging?
Which structures are visualised using Green light in Narrow Band Imaging?
๐ง Equipment Required
1- NBI compatible endoscope, rigid or flexible
2- Light source with optical filters, which isolate the blue and green wavelengths
3- Video processor system
4- High definition monitor
5- Image recording system
๐ Procedure
โญ No staining is required, unlike contact endoscopy.
1๏ธโฃ Perform a standard endoscopy with white light
2๏ธโฃ Identify the suspicious lesions
3๏ธโฃ Switch to the narrow band imaging mode
4๏ธโฃ Observe the vascular patterns and the morphology of the intraepithelial papillary capillary loops (IPCLs)
What are the Applications of Narrow Band Imaging in ENT?
How will you differentiate between Tumour Recurrence vs Post Radiation Swelling using Narrow band imaging?
๐ฌ Pattern Interpretation on Narrow Band Imaging
| Condition | Vascular Pattern |
|---|---|
| Normal mucosa | Regular, thin, uniform capillary loops |
| Inflammation | Dilated but organized vessels |
| Dysplasia | Irregularly enlarged IPCLs, with loss of uniformity |
| Malignancy | Chaotic vascular patterns, tortuous corkscrew vessels and neoangiogenesis |
What are the Advantages of Narrow Band Imaging?
What are the Limitations of Narrow Band Imaging?
๐ง Viva Pearls
- Narrow band imaging is an optical image enhancement endoscopic technique using filtered light.
- IPCL stands for Intraepithelial Papillary Capillary Loop.
- Brown dots represent expanded IPCLs due to tumour neoangiogenesis.
- Narrow band imaging requires no dye.
- Developed in the early 2000s by Olympus Medical Systems, initially for gastrointestinal endoscopy.
- Benign laryngeal lesions show vessels running parallel to the mucosal surface without brown dots.
- Malignant lesions show well demarcated brownish areas with scattered brown dots.
