Interestingly, there are **two different Ice Cream Cone Signs**: 1. A **normal anatomical finding** seen on CT Temporal Bone. 2. A **pathological finding** seen in Acoustic Neuroma (Vestibular Schwannoma) on MRI. In this post, we will discuss both of them in detail. --- ### Ice Cream Cone Sign in Normal Temporal Bone #### 📌 Type of Sign 👉 **Radiological sign** Seen on CT Scan of Temporal Bone. #### 🔍 What is the Sign? The normal ossicular anatomy creates an appearance resembling an ice cream cone. - **Ice cream Scoop** = Head of Malleus - **Ice cream Cone** = Body of Incus Together they produce the characteristic **Ice Cream Cone Appearance**. #### 📍 Where is it Seen? Seen on **Axial CT section of the Temporal Bone**. Specifically at the **level of Epitympanum**. #### 🧠 Why Does it Occur? The appearance is produced by: - Ice cream Scoop is formed by the Head of Malleus - Ice cream Cone is formed by Body of Incus The tapering part of the cone is formed by **Short process of incus** which points toward the **Aditus ad Antrum**. #### ⭐ Clinical Importance The normal Ice Cream Cone appearance confirms: ✅ **Intact Incudomalleolar Joint** Loss of this appearance suggests **Incudomalleolar Disarticulation** --- ### Ice Cream Cone Sign in Acoustic Neuroma #### 📌 Type of Sign 👉 **Radiological sign** Seen on **MRI Brain**. #### 🔍 What is the Sign? An Acoustic Neuroma (Vestibular Schwannoma) produces a characteristic Ice Cream Cone appearance. **Components** - **Ice cream Scoop** = Tumor Component in Cerebellopontine Angle (CPA) - **Ice cream Cone** = Tumor Component in Internal Auditory Canal (IAC) Together they form the characteristic **Ice Cream Cone Sign**. #### 🦠 Seen In Vestibular Schwannoma (Also called Acoustic Neuroma) #### 🧠 Why Does it Occur? - Vestibular Schwannoma arises from **Schwann Cells of the Vestibular Nerve**. - The tumor originates **within the Internal Auditory Canal**. - As it enlarges, it extends outward into the **Cerebellopontine Angle**. ***This produces Narrow intracanalicular component (cone) & Expanded CPA component (scoop), creating the Ice Cream Cone appearance.*** #### 🍦 Why is it Called Ice Cream Cone Sign?
